To become a fully fledged doctor, being able to cure up to 96 people every day (6 per hour), one needs 6 things. The first four are skills:

Now, the good part. First, get 5 stars in Basic medicine. That's after you spend no less than US$4,000 on it. Now, get five stars in Pharmacology. This one is going to cost you no less than US$50,000. After this, you'll need 5 stars in Professional Medicine. This one is no less than US$35,000. And finally 5 stars in Advanced Pharmacology. This is the real killer at no less than US$80,000. Your best bet is to either make it in the music industry first, or have a city or someone rich be your benifactor. If you ask a city, they'll give you the books so long as they don't have enough doctors already and so long as you stay in the city full time and cure people and do nothing else, which can be a good way to work on skills and songs until you're good enough to get into music. The rich benefactor wont put any restrictions on you, but you'll be very very lucky if you find one.
Now, once you're fully skilled in all those things. Which you should be because even then there isn't a 100% strike rate. Go to a hospital and buy a blood sampler and a base medicine. They'll be a lot cheaper, usually at less than US$500 for the pair. And the blood sampler can be used five times.
Now, go to a sick person's interact screen(Make sure that you don't interact with them). Under the regular interactions, there should be a 'use item' drop down list. Use the blood sampler and leave that screen. Now, head back to your own character page. It should say there if you succeeded or not. If you did, go to your item page. Under Medicines, you should have a vial of blood. It'll tell you the strain that the person has. Make sure here that if you've tested two people that you don't get them mixed up! Now, use the vial of blood and hopefully, you'll create some medicine. It'll name the medicine after the strain, so it is possible to keep stocks of the stuff.
Then give it to the ill person and make them use it in their item list.That's it! Of course, you can ask what you want, but many doctors act for free. I ask one out of every couple of patients to buy me new supplies. That's it.

On the 22-Feb-2008 the new (connected with the doctor achievements) way of curing the diseases was implemented. From Developer News:
Buy a Box of syringes down at the hospital. Create the medicine like always. View the medicine details and use the box of syringes to create a shot of medicine. Interact with the patient using the shot. The effects will be instant and if the patient is cured you'll get the achievement.
